Our centers have a dedicated Patient Navigator who works closely with families to make scheduling smooth, easy, and stress-free. They serve as the main point of contact for patient and providers, assisting with appointment coordination, gathering necessary documents, and ensuring clear communication every step of the way.
Patient Referral Received
The patient visits the dentist, confirms the need for surgery, and submits a referral to Valleygate.

Patient Navigator Intro
The Patient navigator reaches out to begin preparing for surgery, ensure the parent has scheduled the surgery clearance appointment with the patient’s primary care provider (offering assistance if needed), and confirms the surgery day.

Pre-admission Requirement
A Pre-Admission History and Physical (H&P) must be completed by the child’s Primary Care Physician within 30 days of surgery and faxed to the Surgery Center for review.
The Patient Navigator will then coordinate with any specialists involved in the child’s care and request all documents needed for anesthesiologists and nursing review.
At this step, we will collect the following information:
- Medical insurance information
- Patient/parent identification
- Completed History and Physical form from the PCP
- Specialist notes as applicable (e.g., cardiology, pulmonology)

Document Management
The Patient Navigator uploads all received records to the patient’s chart to ensure complete and accurate documentation.

Ongoing Communication
The Patient Navigator stays in touch with parents, answers questions, provides directions to the Surgery Center, sends reminders about eating and drinking prior to surgery, and assists with any payment arrangements.

Pre-Surgery Call
A Valleygate anesthesiologist or pre-admission nurse will contact each family one week before surgery to review child’s medical history, answer their questions, and cover essential pre-surgery and home-care instructions. The day before surgery, we provide the arrival time and a final check-in to endure families feel supported and informed.

Surgery Day
The provider meets their patient at the surgery center to perform the scheduled procedure.
